What is all that grunge on and around the chamber and breach face? Looks like grease and/or unburnt powder. That could be part of the issue. I recommend not shooting lead bullets through your GLOCK. Lead bullets are for CAS guns.

Check your extractor and extractor spring for damage and proper function. Also check your ejector for the same thing. I am assuming that your powder charges are consistent because if they are not weak charges could cause this kind of thing including failure to slide lock back after last round. Lastly your main recoil spring on the guide rod should be checked if the above are not the problem. Best of luck figuring it out.
